Participation Agreement

This agreement acknowledges that TILT Forward, under the Association for Enterprise Opportunity, has received the deposit sum of $100 from the Applicant.

This monetary sum shall be held as a deposit for participation in the TILT Forward Bench Pilot Program until the Applicant completes the 6-month pilot.

If the Applicant’s nomination for the program is unsuccessful, the deposit sum shall be returned in its entirety to the Applicant.

If the Applicant is accepted into the pilot program, the deposit will be held for the duration of the pilot and returned upon the Applicant’s successful completion of the following:

6-months continuous use of Bench
Intake survey
3-month check-in interview
End-of-pilot survey and interview

If the Applicant does not complete any of the above requirements, the deposit sum will not be refunded to the Applicant.
